Dear Patient/Parent/Guardian, the information you provide here will help your provider in identifying your/your child’s needs and how to best serve you/your family. Please upload these documents to your spruce health account, or email them to admin@growingmindsmhs.com at least 48 business hours prior to your first intake session:

1. Completed new patient registration forms below. 
2. Photocopy of your photo ID (Driver’s license or passport), insurance card (front and back), and a credit card for us to store on file. 
3. Current medication list and most recent medical records if applicable.
4. Any previous psychological testing report, school IEP evaluation report if applicable.
5. Any guardianship legal documents if applicable.

If you have any questions or need to reschedule, please call (650)-727-1523.  If you need to cancel or reschedule your appointment, please give us at least 48 hours’ notice to avoid no show/late cancellation fee.
