We offer three convenient ways to register as a new patient and schedule your first appointment.

option 1. Online Self-Scheduling (Fastest)

Visit our website at www.growingmindsmhs.com and use the chatbot/autoboot located in the bottom right corner.

Check real-time provider availability.
Select your preferred appointment slot.
Next Step: Once we receive your request, our team will contact you to provide practice details and finalize your registration.

option 2. Phone Registration

Call our office at 650-727-1523. Please have the following information ready:

Full Name and Gender
Date of Birth (DOB)
Insurance Member ID
Next Step: We will create your account and send a Patient Portal Registration link to your email from Charm EHR.

option 3. Email Registration

Send an email to admin@growingmindsmhs.com with your Name, Gender, DOB, Insurance ID, and your preferred provider.

⚠️ Important Notice for All New Patients

To secure your scheduled time, please ensure all New Patient Registration and Intake Forms (Adult or Child) are completed and signed at least 48 hours prior to your appointment.

Note: If forms are not completed by this deadline, we may not be able to hold your appointment slot.

If you encounter any issues with the portal or have questions about the forms, please contact us immediately.

Some Important Reminders

Dear Patient/Parent/Guardian, the information you provide here will help your provider in identifying your/your child’s needs and how to best serve you/your family.

Please upload these documents to your spruce health account, or email them to admin@growingmindsmhs.com at least 48 business hours prior to your first intake session:

Completed new patient registration forms below.
Photocopy of your insurance card (front and back)
Current medication list and most recent medical records if applicable.
Any previous psychological testing report, school IEP evaluation report if applicable.
Any guardianship legal documents if applicable.

If you have any questions or need to reschedule, please call (650)-727-1523. If you need to cancel or reschedule your appointment, please give us at least 48 hours’ notice to avoid no show/late cancellation fee.
